    There is no fixed price for physiotherapy clinic marketing in Australia. What a clinic pays depends on how many suburbs it needs to cover, how much of the work is repair versus growth, and whether paid campaigns are running alongside search and content work. This guide sets out the real cost drivers, how to budget by clinic size, and how to read an agency proposal without being sold a template.

    What actually drives the cost

    A handful of factors explain most of the variance between quotes.

    Channel mix. Local search work, a website rebuild, Google Ads and a compliant review system are distinct workstreams with distinct costs. A single-clinician clinic needing local visibility is buying far less than a multi-practitioner clinic launching paid campaigns across several condition types. Ad spend and management fees are also separate line items — a quote that blends them together is obscuring one of the two.

    Number of locations. A clinic with one site needs one well-maintained Google Business Profile and a set of condition pages. A group with several sites across a city needs a profile per location, suburb-specific pages that do not compete against each other, and reporting split by clinic. That is substantially more work than a single-site engagement.

    Competition and funding mix. Physiotherapy clinics compete for patients coming through several pathways — private health, workers compensation, NDIS-funded supports and DVA arrangements — and each pathway has different search behaviour. Clinics in dense metropolitan catchments with several competing practices should expect higher paid search costs than clinics in regional areas with less competition.

    Starting point. A clinic with a slow, outdated website and a neglected Google Business Profile spends its first months on repair rather than growth. A clinic that already has a functioning site and an active profile can move to acquisition work sooner and typically spends less to reach the same result.

    Budgeting by clinic size

    Rather than quoting numbers that would be wrong for most readers, it is more useful to think in proportions.

    • Single-practitioner clinic, one catchment. Prioritise the Google Business Profile, a small set of strong condition and service pages, and a compliant process for gathering reviews. Paid search can start small and be tightly targeted to your immediate suburbs.
    • Established clinic, several practitioners. Split spend between organic content that compounds over time and paid search that fills the calendar this quarter. This is also the point where condition-specific landing pages start to earn their keep.
    • Multi-site group. Budget needs to cover per-location profile management, suburb-level reporting, and campaign structures that keep spend in the right postcodes rather than cannibalising between sites.

    A useful exercise before agreeing any budget: work out what a new patient is worth to the clinic across a typical course of treatment. If that number is not known, establishing it should come before any spending decision.

    What the first 90 days should look like

    The first quarter of any physiotherapy marketing engagement is largely foundation work, and a straightforward agency will say so upfront. Expect the Google Business Profile to be corrected and rebuilt with accurate categories and services, condition-specific pages written or rewritten, tracking installed so calls and bookings are properly attributed, and — where paid search is part of the plan — campaigns launched around a tight keyword set with a negative list that grows from week one.

    Within 90 days you should see movement in leading indicators: profile views, direction requests, call volume, and a growing number of reviews. Settled organic rankings usually take longer than a single quarter in a competitive catchment, so treat any promise of a specific ranking by a specific date with caution and ask what happens if it is not met.

    How to judge an agency quote

    Look past the deliverables list for four things.

    • Separated media spend and management fees. You should be able to see what goes to Google and what goes to the agency.
    • Named KPIs with dates. "Improve visibility" is not measurable. New patient enquiries per month, calls from the profile and rankings on named condition queries are.
    • Ownership of assets. The website, ad account, analytics property and Google Business Profile should sit in the clinic's name, not the agency's.
    • Compliance literacy. Physiotherapists advertising under the National Law have obligations that generalist agencies often overlook. A proposal that never mentions testimonials, claim substantiation or scope of practice is leaving that risk with the clinic.

    Where the money is usually wasted

The third is a Google Business Profile that was set up once and never touched again, while a competing clinic nearby posts regularly and answers reviews. None of these are budget problems — they are structural ones that a larger budget only makes more expensive.

    Contracts, retainers and month-to-month arrangements

    Agencies structure fees in a few common ways, and each suits a different stage of clinic growth. A flat monthly retainer covering a defined scope of work is easiest to budget against, provided the scope is written down rather than left as a vague list of "ongoing SEO and social." Project-based pricing suits a one-off website rebuild or a Google Business Profile clean-up, but is a poor fit for anything that needs continuous attention, such as paid search management. Performance-linked fees exist in this market but are worth scrutinising closely — a fee tied to lead volume rather than lead quality can incentivise an agency to pursue cheap, low-intent clicks rather than patients who actually book and stay for a course of treatment.

    Whatever the structure, ask what the notice period is and whether there is a lock-in term. A clinic locked into a 12-month contract with an agency that is not performing has limited recourse beyond waiting it out.     In-house, freelance or agency, and seasonal budget adjustments

    Some clinics consider hiring an in-house marketing coordinator instead of engaging an agency, and for a large multi-site group this can make sense once volume justifies a full-time role. For a single-site or two-site clinic, the maths rarely works: a coordinator's salary alone often exceeds what an agency retainer would cost, and one person is unlikely to have deep expertise across SEO, paid search, web development and compliance simultaneously. Freelancers sit in between — often cheaper per hour, but the clinic carries the coordination overhead and the risk of a single point of failure if that freelancer becomes unavailable.

    An agency spreads specialist expertise across a team, which matters in physiotherapy marketing specifically because the work spans several distinct disciplines: technical website work, content written to avoid AHPRA compliance issues, paid search structured around funding pathways, and profile management that needs consistent attention. Clinics that try to run all of this through one generalist hire often end up outsourcing the parts that person cannot do anyway, which erodes the apparent cost saving.

    Physiotherapy demand is not flat across the year. Enquiry volume for sports injuries tends to track school and club sporting seasons, while post-surgical rehabilitation referrals often follow elective surgery scheduling patterns that can shift around public holiday periods. A fixed monthly ad budget applied uniformly across the year will overspend in quiet periods and underspend when demand is highest. Reviewing spend allocation quarterly against actual enquiry patterns, rather than leaving a media budget on autopilot, tends to produce a better return without necessarily increasing the total annual spend.

    The same logic applies to content and profile activity. A clinic near a running club can time content and Google Business Profile posts around race calendars; a clinic near a football club can do the same around pre-season training. This kind of planning costs little beyond the time to think it through, but it changes how efficiently a fixed budget performs.
